If a person actually has hypertension, there is nothing benign about it. The term "neurotic malaise" suggests an emotional or psychiatric connection. A person can go through a psychiatric process called "conversion". This is the process of unconsciously transforming psychic conflict into a somatic symptom. The fact that you mention the terms "benign hypertensive heart disease" and neurotic malaise" indicates whoever examined the person you love, suspected this.
