Organic means the heart muscle is diseased as opposed to having something like BP effect the heart indirectly

There isn't necessarily a difference between organic heart disease and degenerative heart disease. Organic diseases are contained within the affected organ, which means there is some type of abnormality in the organ (which is the heart in this case). Abnormalities may be present at birth, or they may develop from some type of injury that doesn't heal completely. If the abnormality affects organ function and becomes worse over time, it is a degenerative disease. You could have heart disease that is both organic and degenerative, a degenerative disease that is not organic, or an organic disease that is not degenerative.

There are two types of Organic H.D. - Congenital and Rheumatic. Defects that occur at birth are classed as Congenital Heart Disease. These may affect the heart itself : it may not have developed normally during pregnancy, the wall of the heart may be damaged (hole in the heart), or the blood vessels may be underdeveloped. These defects may be hereditary or more likely have been caused by external factors such as drugs or infection during pregnancy. They are normally diagnosed at birth or in early childhood but it is not uncommon for the symptoms to occur for the first time in adulthood. Rheumatic Heart Disease can be the result of a bout of rheumatic fever. Occurrences have decreased considerably due to the use of antibiotics to treat rheumatic fever. There are at least eight specific diseases, which fall into the category of Degenerative Heart Disease. The common factors within this category are that the disease has progressed gradually and that there is no specific event that has caused it.

I am afraid I can see no point in the prefix organic in this context. It is usually applied to distinguish between physical and psychological conditions. I suppose it might mean that a patient has a physical explanation for chest pain, as distinct from a non-physical or psychological one. It might be that you have taken the 2 words out of context. If a physician tested a patient with chest pain and found no identifiable cause,he might say 'there is no evidence or organic heart disease.
